Medicare Advantage Portal Access - Effective December 15, 2025

Beginning December 15, 2025, the Vivant Health Provider Portal will support both Medi-Cal and Medicare Advantage lines of business.

Access Instructions:
  1. Continue to log in at https://eznet.vivanthealth.com/ez-net60/login.aspx
  2. Use your existing Medi-Cal credentials - no new registration required.
  3. For new users, email providerservices@vivanthealth.com to request access.

  • When you log in to EZ-NET, you will be prompted to enter a passcode.
  • EZ-NET will send you an email with a 6-digit code.
  • Enter the code from the email. Ignore the QR code on the EZ-NET screen.
  • Each time you attempt to log in, you will be sent a new code.
